Take Charge of Your Health
Take Charge of Your Health teaches people with chronic conditions management their health care and life-style choices. Chronic conditions include high blood pressure, diabetes, heart conditions, breathing issues, walking problems, and more.
People with different chronic conditions problems attend together. Classes are facilitated by two trained leaders, one or both of whom have chronic conditions themselves. Medical advice is not part of the program.

Each participant in these classes receives a copy of the companion book, Living a Healthy Life With Chronic Conditions.
It is the process in which the program is taught that makes Take Charge of Your Health classes effective. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
This self-help program is taught in six classes, once per week for two-hours each week. Attendance at five of the six classes is recommended to gain the most insight into how to manage health conditions effectively.
Participants learn how to manage day-to-day with their condition, including developing goals and objectives that best fit their personal condition and life-style outcomes. Medical advice is not a part of this program.
